Because Chinese medicine views each person holistically, your medical history and the details of your current health status are extremely important.  Please complete the intake form and informed consent and bring them to your initial visit.  If you have had any extensive medical care (surgeries, long-term treatments, etc.) or are taking multiple medications and/or supplements, please write that history down on a separate sheet as well.  A signed copy of the informed consent is necessary before beginning treatment.
